Enter Doctor’s Name (First, Last)

Enter Zip Code & Coverage Date

Enter Zip Code & Doctor’s Name

You can create a Doctor’s List by clicking “Add”

Enter Zip Code & Choose Distance

For Provide Type, Choose “Primary Care Physician” or “Specialist”

Enter Provider’s First Name & Last Name and Choose “View Results”

Enter Zip Code

Select “Gold Plus HMO/SNP” or “Medicare PPO”

Search by Name

 Enter the doctor’s name, “last name, first name”; if shows up in drop-down list, click on it.  If not, just search anyways.  Sometimes the doctor may still be in-network.
